John Livesley (bio)A disquieting feature of contemporary psychiatric nosology is the tendency to adopt positions that imply that current classifications are simply statements of fact. Clinicians and researchers alike seem to assume that the DSM diagnostic concepts are factual descriptions based only on scientific analysis that reflect the essential nature of psychiatric disorders. The architects of the DSM acknowledge in various ways that this (...) is not the case. Frances et al. (1991), for example, noted the provisional and pragmatic nature of DSM-IV. Nevertheless, these cautions are largely ignored. The DSM is partly responsible. The assertion that the DSM is atheoretical with regard to etiology, although clearly incorrect, tends to generalize to other aspects of the system. Similarly, the emphasis on description that has proved so valuable in improving diagnosis is presented as if clinical description is unencumbered by values and assumptions. Moreover, the failure to state explicitly the assumptions underlying the classification encourages the conclusion that the system is free from theoretical and other influences.In fact the DSM is theory laden. Implicit theoretical models and etiological concepts lie behind many diagnoses, including adjustment, dissociative, and posttraumatic stress disorders. Assumptions about the nature of mental disorders and the way psychopathology is organized underlie the entire system (Livesley and Jackson 1992). Pragmatic and political considerations influenced the final structure. Moreover, as Dr. Sadler and others working on the same issues (Fulford 1989, 1994; Agich 1994) point out, the DSM is also value laden. It is value laden not only in the obvious sense that some diagnostic concepts are highly evaluative or biased against certain groups (Kaplan 1983), but also in the fundamental sense that values are built into the very structure of the system (Agich 1994; Russell 1994; Wakefield 1992).Value influences begin with the definition of mental disorder. As Wakefield (1992) and others have argued, the concept of disorder cannot be value free. Values are also involved in the descriptive terms adopted as diagnostic criteria (Agich 1994) and, as Dr. Sadler convincingly demonstrates, they are intrinsic to the way that the classification is constructed and evaluated—the very aspects of the classification that are generally assumed to be value free. By emphasizing this aspect, Dr. Sadler draws attention to the fact that despite being implicit and informal, these values exert a major influence. They influence kinds of information considered relevant to the classification, the way this information is weighted and evaluated, and the way it is organized into diagnostic concepts. Thus, values shape the classification and the research agenda. This point has not [End Page 223] been fully recognized, or at least not fully acknowledged, by those responsible for developing contemporary classifications. There is a tendency to assume that any values involved in the DSM are largely evaluative and incidental, as if they crept into the system inadvertently due to our limited knowledge. The implication is that their presence is merely a temporary embarrassment to be eliminated automatically as clinical description and phenomenology are refined. But this is not the case; any classification of mental disorders inevitably consists of fact plus value (Fulford 1994). Acknowledgment of the evaluative element does not detract from the achievements of the DSM. It is not a limitation but an essential component.The general failure to attend to the non-factual or non-descriptive aspects of classification means that important aspects of classifications are neglected. The impact of pragmatic considerations, political and economic factors, and ethical and epistemic values on the organization and evaluation of scientific information has not received appropriate attention given the social, clinical, and scientific consequences of contemporary psychiatric classifications. In essence, only a limited aspect of classification has been studied, and even this has been studied in isolation from its context. Under these circumstances there is much to be gained from explicit acknowledgment and analysis of the value component. We are more likely to make progress in resolving nosological issues if we take a broad perspective.For the nosologist, Dr. Sadler makes two useful contributions to understanding the broader view. This volume is intended for use in an undergraduate philosophy course employing the problems' approach. Chapter I provides a clear presentation of Cartesian rationalism. Following the exposition of Descartes' position, there is a section on the standard criticisms levelled by B. Russell. Aune defends the rationalist position with an outline of the traditional arguments for the validity of intuitive knowledge. Chapter I terminates with a list of "Study Questions" and an annotated bibliography suggesting further readings. Chapter II considers classical English (...) empiricism, taking the thought of Hume as paradigmatic. There is a discussion of a priori and a posteriori knowledge as well as a section on the problems of induction and solipsism. Chapter III outlines the attempts of contemporary empiricists to eschew Hume's skepticism by envisioning the world as a construct of our sensory experiences. To avoid the solipsism still implicit in the latter position, a re-evaluation of Hume's theory of perception is offered. Wittgenstein and Strawson are briefly treated in this section. Chapter IV explains the pragmatist's attempt to reduce to the same untenable position the rationalist and empiricist views of the a priori. Chapter V concentrates on the pragmatic theory of justification and includes a treatment of N. Goodman's "new riddle of induction." Aune's book is a readable account of conflicting theories of knowledge and undergraduates should find it of value.--W. J. L. (shrink)

Peter Winch's The Idea of a Social Science has been the subject of repeated misunderstanding. This discussion takes one recent example and shows how Winch's argument is gravely distorted. What is at issue is not, as is usually supposed, whether we can accept or endorse another society's explanations of its activities, but whether we have to look for an explanatory connection between concepts and action. Winch's argument is that before we can try to explain actions, we have to identify them (...) correctly. This can only be done by seeing how they, and the concepts they are associated with, fit within a way of life. Grasping its rule?following character is understanding action. Once the difficulties in making such identifications are appreciated, we will be less inclined to accept facile explanations why people in other societies do the things they do. (shrink)

Extensive research has demonstrated that rs1360780, a common single nucleotide polymorphism within the FKBP5 gene, interacts with early-life stress in predicting psychopathology. Previous results suggest that carriers of the TT genotype of rs1360780 who were exposed to child abuse show differences in structure and functional activation of emotion-processing brain areas belonging to the salience network. Extending these findings on intermediate phenotypes of psychopathology, we examined if the interaction between rs1360780 and child abuse predicts resting-state functional connectivity (rsFC) between the amygdala (...) and other areas of the salience network. We analyzed data of young European adults from the general population (N = 774; mean age = 18.76 years) who took part in the IMAGEN study. In the absence of main effects of genotype and abuse, a significant interaction effect was observed for rsFC between the right centromedial amygdala and right posterior insula (p <.025, FWE-corrected), which was driven by stronger rsFC in TT allele carriers with a history of abuse. Our results suggest that the TT genotype of rs1360780 may render individuals with a history of abuse more vulnerable to functional changes in communication between brain areas processing emotions and bodily sensations, which could underlie or increase the risk for psychopathology. Worries about safety of approved drugs have pushed post registration research to become the fastest growing drug research phase. Until recently, phase IV studies were mainly conducted for marketing purposes and run much like a phase III trial—at institutions with experienced investigators and a list of inclusion and exclusion criteria. Innovative phase IV studies involve ordinary physicians in research naïve communities. This brings ethical issues familiar to medical research into clinical practice. As a consequence, individual physicians are challenged to protect (...) scientific integrity and to secure the ethical conduct of research. Several ethical issues need to be addressed in the process of developing a scientifically sound and ethically high principled practice of phase IV research.PHASE IVEssential to phase IV research is the focus on how drugs work in the real world. The research goal may be to explore a specific pharmacological effect, to establish the incidence of adverse reactions, or to determine the effects of long-term administration of a drug. They may also be designed to establish a new clinical indication for a drug.1 Subjects may or may not be randomised, and the studies typically involve large numbers of patient-subjects and physicians.Doubts concerning the scientific rigor and clinical value have put postmarketing research in a poor light. However, high quality phase IV research is badly needed to further pharmaceutical science. First, because it can provide reliable data on the effectiveness, safety and optimal use of a drug in treated populations.2 3 In pre-registration clinical trials , a pharmaceutical product is studied under unrealistic circumstances. Under-representation of women and elderly are common and the concomitant use of medicines is usually extremely restrictive in these trials.4 Second, phase IV can provide much needed data for evidence-based …. Sustained attention, a key cognitive skill that improves during childhood and adolescence, tends to be worse in some emotional and behavioural disorders. Sustained attention is typically studied in non-affective task contexts; here, we used a novel task to index performance in affective versus neutral contexts across adolescence (N = 465; ages 11–18). We asked whether: (i) performance would be worse in negative versus neutral task contexts; (ii) performance would improve with age; (iii) affective interference would be greater in younger adolescents; (...) (iv) adolescents at risk for depression and higher in anxiety would show overall worse performance; and (v) would show differential performance in negative contexts. Results indicated that participants performed more poorly in negative contexts and showed age-related performance improvements. Those at risk of depression performed more poorly than those at lower risk. However, there was no difference between groups as a result of affective context. For anxiety there was no difference in performance as a function of severity. However, those with higher anxiety showed less variance in their reaction times to negative stimuli than those with lower anxiety. One interpretation is that moderate levels of emotional arousal associated with anxiety make individuals less susceptible to the distracting effects of negative stimuli. (shrink)

Decentralized clinical trials (DCTs) have the potential to advance the conduct of clinical trials, but raise several ethical issues, including obtaining valid informed consent. The debate on the ethical issues resulting from digitalization is predominantly focused on direct risks relating to for example data protection, safety, and data quality. We submit however, that a broader view on ethical aspects of DCTs is needed to touch upon the new challenges that come with the DCT practice. Digitalization has impacts that go beyond (...) its direct purposes, by shaping behaviors, experiences, social relations, and values. We examine four elements of the informed consent procedure that are affected by DCTs, while taking these soft impacts of technologies into account: (i) informing participants and testing understanding, (ii) freedoms in relation to responsibilities and burdens, (iii) trust in participant-researcher relations, and (iv) impacts on the concept of privacy. Our analysis reveals that a broad view is key for optimal conduct of DCTs. In addition, it provides insight into the ethical impacts of DCTs on informed consent. Technologies such as DCTs potentially have profound impacts which are not immediately addressed by the existing regulatory frameworks, but nonetheless important to recognize. These findings can guide future practices of DCTs to foster the important values of clinical research in this novel approach for conducting clinical trials. (shrink)

There exist important deductive systems, such as the non-normal modal logics, that are not proper subjects of classical algebraic logic in the sense that their metatheory cannot be reduced to the equational metatheory of any particular class of algebras. Nevertheless, most of these systems are amenable to the methods of universal algebra when applied to the matrix models of the system. In the present paper we consider a wide class of deductive systems of this kind called protoalgebraic logics. These include (...) almost all (non-pathological) systems of prepositional logic that have occurred in the literature. The relationship between the metatheory of a protoalgebraic logic and its matrix models is studied. The following results are obtained for any finite matrix model U of a filter-distributive protoalgebraic logic : (I) The extension U of is finitely axiomatized (provided has only finitely many inference rules); (II) U has only finitely many extensions. We study modal logics in the setting of varieties of modal algebras. Any variety of modal algebras generated by a finite algebra — such, a variety is called tabular — has only finitely many subvarieties, i.e. is of finite height. The converse does not hold in general. It is shown that the converse does hold in the lattice of varieties of K4-algebras. Hence the lower part of this lattice consists of tabular varieties only. We proceed to show that there is (...) a continuum of pretabular varieties of K4-algebras — those are the non-tabular varieties all of whose proper subvarieties are tabular — in contrast with Maksimova's result that there are only five pretabular varieties of S4-algebras. A well-known result, going back to the twenties, states that, under some reasonable assumptions, any logic can be characterized as the set of formulas satisfied by a matrix 〈,F〉, whereis an algebra of the appropriate type, andFa subset of the domain of, called the set of designated elements. In particular, every quasi-classical modal logic—a set of modal formulas, containing the smallest classical modal logicE, which is closed under the inference rules of substitution and modus ponens—is characterized by such a matrix, (...) wherenow is a modal algebra, andFis a filter of. If the modal logic is in fact normal, then we can do away with the filter; we can study normal modal logics in the setting of varieties of modal algebras. This point of view was adopted already quite explicitly in McKinsey and Tarski [8]. The observation that the lattice of normal modal logics is dually isomorphic to the lattice of subvarieties of a variety of modal algebras paved the road for an algebraic study of normal modal logics. The algebraic approach made available some general results from Universal Algebra, notably those obtained by Jónsson [6], and thereby was able to contribute new insights in the realm of normal modal logics [2], [3], [4], [10].The requirement that a modal logic be normal is rather a severe one, however, and many of the systems which have been considered in the literature do not meet it. For instance, of the five celebrated modal systems, S1–S5, introduced by Lewis, S4 and S5 are the only normal ones, while only SI fails to be quasi-classical. The purpose of this paper is to generalize the algebraic approach so as to be applicable not just to normal modal logics, but to quasi-classical modal logics in general. (shrink)

Professor J. B. Schneewind's recent excellent volume Mill's Ethical Writings has drawn attention to the necessity of studying Mill's notes to chapter XXIII of his father's Analysis of the Phenomena of the Human Mind for a clear understanding of his theory of the moral sentiments. There are notes, however, by J. S. Mill to other chapters of that work, which should not be forgotten, because they elucidate the associationist theory of motivation which is obscurely appealed to in chapter IV of (...) Utilitarianism. Critics of Mill from F. H. Bradley onwards have made erroneous statements about the psychology that underlies Mill's justification of the principle of utility. But if Mill was going to support his ethics with a psychological theory, he should have expressed that theory at length and unambiguously in his ethical treatise. In what follows, it is pointed out where critics have been unfair to Mill through their apparent ignorance of his notes to the Analysis and of his conception of sympathy; but it is also shown that there is a puzzling inconsistency between the notes and a key passage in Utilitarianism, chapter IV. Finally, the theory of habitual volition, which is expressed at the close of that chapter, is examined. Critics have paid little attention to this theory, but it entails a serious modification of psychological hedonism, and Mill obviously attributed great importance to it. (shrink)

On the History of Modern Philosophy is a key transitional text in the history of European philosophy. In it, F. W. J. Schelling surveys philosophy from Descartes to German Idealism and shows why the Idealist project is ultimately doomed to failure. The lectures trace the path of philosophy from Descartes through Spinoza, Leibniz, Kant, Fichte, Jacobi, to Hegel and Schelling's own work. The extensive critiques of Hegel prefigure many of the arguments to be found in Feuerbach, Kierkegaard, Marx, Nietzsche, Heidegger, (...) and Derrida. This is the first English translation of On the History of Modern Philosophy. In his introduction Andrew Bowie sets the work in the context of Schelling's career and clarifies its philosophical issues. The translation will be of special interest to philosophers, intellectual historians, literary theorists, and theologians. (shrink)
